Please print and fill out the form and return it to Dr. Venkat Subramaniam, Department of Computer Science, University of Houston, Houston, TX 77204-3475. You may fill it and turn it in to the person coordinating the course at your site as well. This form must be signed by Monday January 22nd, 2001. If you have not taken the course at UH and would like to continue, you need to discuss with Venkat.

Those on campus should see me during my office hours on Monday. Those on remote site may communicate with me by email. (When you write to me about pre-req. please put your name and indicate which location you are registered at).

You are responsible to drop the course by the drop date if you do not meet the pre-req.

This form is intended for the students registered at remote sites. The students who attend the class on Monday Jan. 22nd at the studio in main campus will be provided this form in class.
